Had a similar thing happen here in VA...the guy gave insurance info to the police that turned out to be bogus, my insurance company just filed the paperwork and processed it as uninsured. I was told that this was the quickest way to get my car fixed and wake up the individual since DMV will suspend the license and all vehicles registered if they can't provide proof of insurance in 30 days. We allow uninsured drivers in VA, but they have to pay an additional $500 fee/yr/car to register. I would either file the form ASAP or get your insurance company to do it.

I just found this while looking for another thread. In the end, I forgot to mail the form to get the owners license suspended (dumb) and my insurance company sent me a letter recently to say that they recovered $0 after all this time and were closing out the case. Thanks and tough luck...
